Addiction is a behavioural condition — compulsive use continuing despite harm. Conflating it with dependence causes real damage: patients who need pain relief turn down it out of fear, and others assume that owing to the fact that withdrawal happened they must be addicted. Being physically dependent on a medicine you take as prescribed is not a moral event, it is pharmacology.

The Combinations That Actually Cause Harm

Opioid deaths are rarely about the opioid alone. The overwhelming majority involve a second central nervous system depressant — alcohol, a benzodiazepine, or a sleeping tablet — owing to the fact that their effects on breathing add together. The dose that was safe yesterday is not safe alongside two glasses of wine tonight.

This matters most in ordinary situations in place of dramatic ones: a prescribed sleeping tablet, a benzodiazepine from a different prescriber, a drink at a family occasion.
