As a "Z-drug" it binds selectively to the receptor subtype governing sedation, so it produces sleep without much of the muscle relaxation and next-day heaviness older hypnotics caused. It is a poor choice for someone who wakes at three, owing to the fact that by then it has gone.

For Staying Asleep

Lunesta (eszopiclone) lasts around six hours and covers both problems, making it a sensible choice when the pattern is mixed — and it is the one hypnotic studied over six months and carrying no recommended cap on treatment length. Restoril (temazepam) is the benzodiazepine option, with eight to twenty hours of cover; it holds the night well and suits patients whose insomnia is driven by anxiety, since benzodiazepines address that directly while Z-drugs largely do not.

The Rules That Matter More Than the Choice

Two apply to everything in this line-up. Only take a sleeping tablet when you can give sleep a full seven to eight hours — taken too late, enough remains at waking to impair driving measurably, and that impairment is notoriously hard to detect in yourself. And the Z-drugs carry a boxed warning for complex sleep behaviours: sleepwalking, sleep-driving, eating or messaging while not fully awake, with no memory afterwards. Rare, but if it happens even once, stop and speak to your prescriber. Take the tablet already in bed with the lights off; staying up afterwards is how most such episodes begin.

Rebound Insomnia Is Not Relapse

A night or two of worse-than-usual sleep after stopping is rebound insomnia — expected, temporary, and not evidence that the original problem has returned. Misreading it is the commonest reason a two-week course becomes a two-year one. Plan the stop for a stretch without important early mornings, expect a couple of unremarkable nights, and taper in place of stopping abruptly after a long course.

When the Real Problem Is Something Else

Insomnia is often a symptom in place of a diagnosis. Untreated sleep apnoea, restless legs, chronic pain, thyroid disease, anxiety and depression all present as poor sleep, and medicating the sleep leaves the driver in place. Loud snoring with witnessed pauses, or waking unrefreshed after a full night, deserves investigation. Alcohol deserves specific mention: it shortens the time to fall asleep and then fragments the second half of the night, so a nightcap reliably buys an easier bedtime at the cost of a worse night.
